The mother of my daughter left her with me 2 months ago, she moved, quit her job and disconnected her number, she pretty much jus left her life for good. How do I file any type of paper work to remove her mother from any and all parental rights? I live in salt lake city ut

You don't. This is not abandonment, nor is it grounds for termination of parental rights. There are sticky notes at the top of this forum that deal with both issues.

What you REALLY want is to get sole legal and physical custody. For this you go to your local Family Court and petition the court for that. But forget about terminating rights. Not going to happen.
