Where can I go to find the forms to print off for my ex husband to sign away his rights to our child?

in most cases, he can't just sign his rights away. he can choose not to have visitation and give you full custody and all that, but he's still responsible for child support and anything else the court decides he should pay for. like maybe health insurance.
