I was married to my daughter's father and we divorced after my daughter was born and he has not been in her life at all, he lives in another country. I have custody of my daughter and he has no visitation of her and I am engaged to be married soon. My fiance is the only father that my daughter has even known she has never seen her real father.

My questions are:
1. Do I have to get consent from my daughter's father in order for my husband to be able to adopt her?
2. If I do have to get his consent for the adoption, do I also have to get consent just to change her last name.
