How might you treat a 1099-MISC that has at the bottom something to the effect

"This is taxable to the amount used for health care premiums".

All of the amount was used to purchase health care premiums. In previous years, it was issued as a 1099-R as non-taxable. How would you list and exclude from income?

Took the note approach.
