I have form 16A. Company paid Indian taxes for me, what form do I neeed for IRS and what do I declare as income?

On the U.S. tax return, you must declare worldwide income. So you will declare India income. You will also file Form 1116 to claim credit for taxes paid in India. You may also be required to file your India tax return as nonresident in India. You will report only your India income.
