iIlive in queens, ny and work in NYC. what tax forms do I fill out and give to my employer? I need to know the proper form numbers. I have already completed federal, but need NYC resident and work in NYC form. Do I also need to fill out NY state?

Please advise.

Form IT-2104 is NY's version of the federal W-4 form. It covers both NY State and NY City. You can find it here:
