If it's that bad and the pain has worsened I would go to the ER but I am no doc. That's just what I would do. Or at least give them a call and see if maybe you can speak to a nurse to find out if you should go in?

What kind of pain by the way? Any pain in your back? It's possible you waited to long to treat the UTI and it has turned into a kidney infection called pyelonephritis. The symptoms are much the same as with a UTI and may be accompanied by back pain and a fever. I AM NOT SAYING YOU HAVE THIS. But it could explain why your pain has worsened rather than improved even with medication. If this has happened you will more than likely just need another antibiotic to clear the infection. I had pylo once and they just gave me a strong antibiotic and it cleared up. But it hurt like you know what until the meds kicked in! They had to give me pain killers until the meds did kick in.
