and with a lot of extra work and wiring, Radio Shack. The switches Radio Shack sells don't have tapped holes, so installation is difficult. Replacement switches have the tapped holes and flying leads.

Plenty of hardware stores have them, as well as the ceiling fan dept of Menards and Lowes. It's a standard DPDT slide switch rated at 120vAC and at least 2 amps.
