I have CD interest from Countrywide and Etrade Bank. Is this interest taxed at 5% or 12%or in-state/out of state bank interest? Both have branches in MA. Thanks.

Heres' an earlier response which indicates that if you can walk into a branch in MA, then you can claim it as in-state bank:
