I have a household employee. I am filling out her w-2. She lives in Kentucky and I live in ohio. Do I need to apply for an employer State Id in Ohio or Kentucky?

Apply for Kentucky.

Kentucky and Ohio have a reciprocity agreement taht says people working in one state and living in the other will file and pay the taxes in their HOME state.
