Writing Minutes
When a motion is made in a meeting and there is discussion but no one seconds the motion, what is the proper format for recording this in the minutes?

Motion failed due to lack of a second. Actually, there should not be discussion prior to a second. If you follow Robert's Rules of Order if there's no second then no one should be allowed to comment.
