Suffixes in names

What is the correct way to type "John M. Doe, Sr." with the last name first. Is it

Doe Sr., John M. or Doe, John M., Sr. or Doe, John M. (Sr.)??

The correct order is 'Surname, First, Middle (or initial), Suffix'
